1. Your original Emirates ID
2. A copy of the passport and a copy of residence visa, if you’re a resident

Those not holding People of Determination card or Sanad card need to provide: 

Citizens are required to provide their original Emirates ID and a medical report not more than three months old with the applicant’s health condition.

• Dubai residence visa

• A medical report stating your health condition and is also not more than three months old.
